1. A written work of approximately 900 words or less 2. The genre or literary form you consider your work to be 3. Any pertinent background information you would like to share about the work 4. A photograph of yourself, either current or past 5. A short bio that includes your full name, date of birth, place of birth. We love to hear about your family, your accomplishments or anything you'd like us to know.
